Women's Hormone Panel: What to Test Before Starting Therapy

Lab work before hormone therapy serves two purposes that are easy to conflate. The first is characterizing the hormonal picture. The second — often more valuable — is ruling out conditions that produce overlapping symptoms and would make hormone therapy the wrong answer. Thyroid dysfunction, iron deficiency, and poorly controlled metabolic disease all present with fatigue, mood change, and cycle irregularity.
This is an educational overview of what providers commonly order and why. Which tests are appropriate, and how to interpret them, is a clinical decision.
Why Timing Changes Everything
In a cycling woman, estradiol and progesterone vary enormously across the month. A progesterone level drawn on day 5 tells you almost nothing; the same test drawn seven days after ovulation tells you whether ovulation occurred.
The standard conventions:

In a post-menopausal woman, or one without a predictable cycle, timing conventions no longer apply and the interpretation changes accordingly.
The Core Panel
Sex Hormones
- Estradiol (E2) — the primary estrogen of the reproductive years. Interpretation depends entirely on cycle phase and menopausal status.
- Progesterone — most informative as confirmation of ovulation rather than as an absolute value.
- FSH and LH — elevated FSH supports a menopausal or perimenopausal picture. In PCOS, an elevated LH:FSH ratio is a classic though inconsistent finding.
- Total and free testosterone — women produce testosterone in clinically meaningful amounts. Low levels are relevant to libido, energy, and lean mass; elevated levels point toward hyperandrogenism.
- SHBG — binds sex hormones and determines how much is free and active. A normal total testosterone with low SHBG can mean elevated free testosterone, so SHBG is what makes the total interpretable.
- DHEA-S — the adrenal androgen precursor. Helps distinguish adrenal from ovarian sources of androgen excess.
Thyroid
A TSH alone is a screen, not a panel. Free T4, free T3, and thyroid antibodies are commonly added because hypothyroidism mimics much of what brings patients in, and autoimmune thyroid disease is substantially more common in women.

Metabolic Markers
Fasting glucose, fasting insulin, HbA1c, and a lipid panel. Fasting insulin is frequently omitted from standard panels and is often the most informative single marker for early insulin resistance — it rises well before glucose or HbA1c move.
General Health Markers
A CBC catches the anemia that explains a great deal of unexplained fatigue. Ferritin is worth ordering specifically, since iron deficiency without anemia is common in menstruating women and produces fatigue at normal hemoglobin. Vitamin D, B12, and a comprehensive metabolic panel round out the baseline.
What Labs Cannot Tell You
Symptoms Do Not Track Numbers
Two women with identical estradiol levels can have entirely different symptom burdens. Receptor sensitivity, metabolic clearance, and individual set points vary. This is why competent providers treat the patient rather than the number, and why a "normal" result does not invalidate a symptom.
Reference Ranges Are Population Ranges
A laboratory reference range describes a statistical distribution of a reference population — not an individual's optimal value. Being within range is not the same as being at your own baseline.
Perimenopause Resists Testing
During the transition, hormone levels fluctuate unpredictably from cycle to cycle and even week to week. A single draw can look entirely normal in a woman with significant symptoms. Perimenopause is diagnosed clinically, from symptom pattern and menstrual history, not from a lab value.
Testing After Starting Therapy
Follow-up testing depends on route. Transdermal, oral, sublingual, and pellet delivery produce different pharmacokinetics, and some routes are poorly reflected by serum testing. Timing relative to the last dose matters as much as the interval since starting. Your provider will specify both — following a generic protocol found online commonly produces uninterpretable results.

Frequently Asked Questions
When in my cycle should I get hormone testing?
Days 2–5 for FSH, LH, and estradiol; roughly seven days after ovulation for progesterone. Testosterone, SHBG, and DHEA-S are best drawn in the morning, when diurnal variation matters more than cycle phase. If you are not cycling predictably, your provider will adjust.
Can I test hormones while on birth control?
Hormonal contraception suppresses endogenous production and alters SHBG, so results reflect the medication rather than baseline function. Providers who need baseline values will discuss whether and how long to discontinue first — which is a clinical decision, not something to do independently.
Is saliva or blood testing better?
Serum testing is the most widely validated and what most clinical guidance is built on. Saliva and dried urine testing are used in some practices and have advocates, particularly for metabolite patterns, but reference standards are less established. Consistency of method matters when tracking over time.
Do I need testing if my symptoms are obvious?
Even with a clear clinical picture, baseline labs screen for conditions that mimic hormonal symptoms — thyroid disease, anemia, and metabolic dysfunction chief among them. That screening function is the main argument for testing.
What is the single most useful test?
There isn't one. If forced to name an underused one, fasting insulin — it is frequently omitted and often the earliest signal of the insulin resistance underlying many presentations.
Why did my provider say my labs are normal when I feel awful?
Reference ranges describe populations, not individuals, and perimenopausal hormone levels fluctuate enough that a single draw can miss the picture entirely. A normal result does not invalidate the symptom, and it is reasonable to ask what else is being considered.
